Drone Laws in Indonesia

Finding clear and concise information regarding drone laws in Indonesia seems to be quite a challenge.(especially in English) I was curious about this as drones are featuring in the media quite frequently here. Below I have compiled a list of rules and regulations that should make things a lot clearer. I would like to state that I have never heard of these rules being enforced except in restricted areas and over large crowds.
Maximum flight altitude: In Indonesia applies a maximum flying height of 150 meters (500 feet). If you want to fly higher, you have to apply for permission at least 14 working days before the flight date.
Maximum horizontal distance: None. (line of sight, as a general rule of thumb)
 Insurance:  Not required.
Maximum take-off weight (MTOW): For drones heavier then 55lbs and for commercial  use such as surveying land you must apply for a permit
Distance to airports: Flights are only permitted in uncontrolled airspace.
Other safe distances: Don't fly over crowds and keep away from built-up areas. (This is quite challenging in places like Jakarta. I recommend morning flights just after sunrise) 


Restricted zones: Do not attempt to fly inside their restricted zones. (recently they have used anti UAV devices around in a number of locations to bring down media drones in restricted areas)
Flight approval: Is not required for hobbyists under 150 meters by law. It is needed for commercial activities. There is a useful PDF here  in relation to what activities that it covers.
Time of operations: The old regulations from 2015 stated that it must be during daylight hours. I am unclear as to what the new regulations are in relation to this. 
Specific regulations: You should note that your drone may approach the land border to a maximum of 500 meters if it is equipped with a camera.
Fines: If you do not comply with the regulations, you are threatened with up to three years of imprisonment and a penalty of more than 66.000 euros. (I have heard of non of these penalties being enforced)
